Il problema dell'angoscia viene affrontato da Lacan da un'angolatura particolare, e cioè dal suo aspetto positivo. In questo ha dei precursori in alcuni filosofi, come Kierkegaard, Heidegger e Sartre stesso, che hanno visto nell'angoscia quel concetto capace di aprire alla riflessione soggettiva, ma soprattutto Freud, che assegna all'angoscia la funzione di causare la rimozione, dando cosi avvio alla formazione del sintomo. Lacan accentua la positività dell'angoscia, poiché per lui essa è la via privilegiata per accedere al reale. Il che vuol dire che l'angoscia è la via che porta al di là del significante, è la via privilegiata per cogliere quel "resto" che egli chiama oggetto "a". Oggetto essenziale, poiché non è l'oggetto desiderato, ma è l'oggetto che, invece, causa il desiderio. Si passa così, tramite l'angoscia, dal campo dell'oggetto docile al significante, come sono tutti gli oggetti che sembrano corrispondere al desiderio, a quest'oggetto "altro", che è all'origine del desiderio. Oggetto le cui manifestazioni erano già state delineate da Freud nell'oggetto orale e in quello anale, e che Lacan a sua volta completa con lo sguardo e la voce, e infine precisando la natura del fallo, oggetto di cui l'uomo viene a mancare e di cui la donna non sa che fare, lei che è invece sempre presa dal desiderio dell'Altro.
